Office News
June is the target for the opening of our new minor procedures suite. We will be offering endometrial ablations, operative hysteroscopy, Essure birth control, labial reduction, and other minor procedures in an office setting. These procedures will be performed under light anesthesia administered by an Anesthesiologist.
The advantages are many:
- Significant cost savings over the outpatient surgery center.
- Simplicity. You can expect to be in and out in less than an hour.
- Comfort and familiarity.
We are excited to be able to offer these in-office procedures that previously necessitated an outpatient surgery center. The Anesthesiologists are the same Anesthesiologists we use in the hospital. They will be supplying all the equipment necessary to ensure your safety and comfort.
Remember that we have Saturday hours from 9 am to 4 pm. Working women are particularly appreciative of the opportunity to be seen on a Saturday. Dr. Wilson, Dr. Spieldoch,and Paula Mazzacano are available for appointments on Saturdays.
We have been offering patients the opportunity to drop in for emergency Gynecologic care for a few months now. If you have an immediate problem, do not hesitate to drop in. You will be seen by a provider as soon as there is an opening. You do not need to be an established patient to take advantage of this.

Viagra Does Help Sexual Dysfunction in Women

Viagra (Sildenafil) has been widely accepted as an agent to treat male erectile dysfunction. Now, it has been shown that some women may also benefit from this medication. Female sexual dysfunction has been divided into four major categories – desire, arousal, orgasm and pain. Over the past few years, there has been slow progress in developing medical interventions for these areas. Decline in ovarian hormone production with aging has a significant effect on the first three categories, and can be improved with administration of appropriate doses of estradiol and testosterone in healthy women. However, many reproductive age (premenopausal) women who take antidepressant medication notice changes in their arousal and orgasm function within the first year of starting the medication – especially the serotonin reuptake inhibitor (SRI) class of drugs.
One well-constructed study recently focused on premenopausal women with normal hormone levels, who had developed sexual dysfunction after being prescribed an SRI antidepressant, despite the fact that their depression had been successfully treated with the medication. After taking Viagra, the women experienced a significant improvement in their sexual function compared to the placebo group. Side effects occurred (headache, flushing, upset stomach) but no one withdrew because of serious adverse events.
Other studies over the past year have shown that Viagra improves the clitoral and uterine blood flow in healthy postmenopausal women as well. This is likely the mechanism responsible for the improvement in sexual function seen in the premenopausal age group, and raises the possibility that Viagra may be useful when prescribed along with hormone therapy in properly selected patients.
It is important to make sure that baseline thyroid function and ovarian hormone levels are optimal, and that any underlying relationship issues are addressed as well, before considering any medical intervention.

Ovarian Cancer Alert
A recent study in the Journal of the American Medical Association shows that women who have ovarian cancer are more likely than those without ovarian cancer to have three specific symptoms with higher frequency prior to diagnosis: abdominal swelling, feeling the urge to urinate and feeling bloated. The study involved 1,709 women who visited a clinic for routine examinations or gynecologic complaints and 128 women with benign or cancerous masses. All were quizzed on whether they felt the symptoms as well as seventeen other symptoms. Forty-three percent of women with ovarian cancer reported those three symptoms up to 20 to 30 times per month. The same symptoms were much less frequent in women who had benign conditions such as irritable bowel syndrome. Study co-author Lynn Mandel,Ph.D., says that a woman who has those symptoms continually for more than three weeks should talk to her doctor, especially if symptom onset is recent.
We recommend pelvic ultrasound for any woman who experiences these symptoms. Generally speaking, ovarian cancer can be detected even in early stages by a pelvic ultrasound.
A pelvic ultrasound is quick, easy, painless, and harmless.
And it can save your life.

Q. How do I know when I need more pellets?
A. Generally speaking, you will feel the pellets wearing off. You will begin to experience hot flashes, night sweats, loss of libido, and insomnia. We encourage you to come in for a blood draw when you notice these symptoms. Even if it is not time for your next blood draw, you are welcome to make an early appointment to check your levels.
Q. When you report my levels to me, you give me numbers. I don't know what these numbers mean. What should I be looking for?
A. The range of normal for estradiol is wide. Generally, just after the pellets are inserted, the level of estradiol can be as high as 300. It then gradually drifts down, and we encourage additional estradiol pellets when the level drops below 100. Testosterone levels can be as high as 200 just after the pellets are inserted, and we recommend more pellets when the levels drop to below 50.
This being said, it is important to remember that women are individuals, and what is a perfect level for one woman may be inadequate for another. Some women feel best when their estradiol levels are at 300, and others feel best at 100. We will work with you to establish the best levels for you.
Q. My friend who lives back east asked her doctor about hormone pellets, and he told her that he doesn't use them because they are not FDA approved and because the levels vary too much. Is this true?
A. Estradiol and Testosterone are FDA approved in many, many different forms. So the drugs contained in the pellets are approved, but the delivery system is not. What this means is that the companies who produce the pellets have not gone through the expense of doing the research to gain FDA approval for this particular delivery system. There are many examples in medicine of this approach.
The levels of estradiol and testosterone do vary in a woman using the pellets, but in a normal reproductive women's cycle the levels also vary tremendously throughout the month. Women do not tend to notice these fluctuations. They only notice when the estradiol and testosterone levels drop and menopausal symptoms resume.

Minimally Invasive Hysterectomies
Did you know that 66% of hysterectomies in the United States are still performed through a large abdominal incision? Minimally invasive surgery is certainly the trend in this area, but it has not permeated other parts of the country, particularly rural areas.
